• What’s involved:

Assessing support needs of service users. Drawing up support plans, implementing, monitoring and reviewing them.

Co-ordinate and maintain the package of support including personally providing support, accessing and liaising with other resources, including:
• acting as a key provider of support services
• advocating on behalf of service users
• welfare benefits advice and budgeting skills
• accessing resources from statutory and voluntary agencies
• developing skills for independent living and successful tenancy sustainment.
• ensuring access to employment and training services
• support service users with Child Protection issues

To liaise and develop effective working relationships with Customer Service department. Ensure resolution to the benefit of the trust and the customers where possible.

Assist service users in accessing appropriate housing. This would include drawing up and implementing resettlement plans and providing follow-up support.

Provide information and consult service users about services provided and actively encourage participation.

Liaise with the Referrals & Assessment Team to maximise take up of service. Ensuring that appropriate information regarding service users is shared & service users’ needs are met.
